With osteoarthritis of the knee grade 1, pain usually occurs only when the knee joint is subjected to strain. The cartilage layer becomes softer but is not yet damaged on the surface. With osteoarthritis of the knee grade 2, people feel pain when moving. Certain risk factors can advance the development of secondary osteoarthritis of the knee and accelerate the progression of the condition.

Risk factors for osteoarthritis include general injuries of the joint structures that are caused by repeated excessive strain or an accident. This means that sports and occupational strain exerting a lot of pressure on the joints can lead to osteoarthritis of the knee. In the same way, pre-existing conditions such as diabetes, gout or necrosis of the femoral head can result in osteoarthritis of the knee.

Another risk factor for osteoarthritis is being overweight or obese. Based on the 2010 GEDA study, a positive linear connection was observed between the development of osteoarthritis of the knee and the Body Mass Index (BMI). Furthermore, various scientific studies established that obesity generally advances the progression of the condition.

Osteoarthritis of the knee can be advanced by joint misalignments and the pressure they exert. A misalignment of the legs can cause one-sided wear. Knock knees, for example, lead to osteoarthritis of the outside of the knee (valgus osteoarthritis), and bandy legs result in osteoarthritis of the inside of the knee (varus osteoarthritis). If such a misalignment exists, the mechanical strain can be minimized with a correction of the leg axis position. This may delay the progression of osteoarthritis.
